Practice management software for California therapists and coaches has to fit a state with its own licensing boards, no psychology compact, a seven-year records rule and a commercial market where Kaiser covers almost half of insured people. California licenses more behavioral health clinicians than almost any state. The Board of Behavioral Sciences reported 58,107 licensed marriage and family therapists, 42,886 licensed clinical social workers and 6,161 licensed professional clinical counselors in its second-quarter FY 2025/26 statistics (BBS board materials, February 2026). A private practice here competes with group practices, Kaiser's integrated system and national telehealth companies, which is why running your own brand on your own domain matters more than in most states.

Licensing boards for California therapists and coaches
California splits mental health licensing across two boards:

Coaching is not a licensed profession in California, but a coach who also holds an LMFT or LCSW license is practicing under that license when they deliver therapy, so the board rules apply to those sessions.

Telehealth rules: compacts and client location
PSYPACT. California is not a member. The Board of Psychology's telehealth FAQ says so directly and explains that, from January 1, 2026, psychologists licensed in another state or in Canada must obtain a Temporary Out-of-State Psychologist License through BreEZe before providing temporary services in California, valid for 30 consecutive days in a calendar year (Board of Psychology telehealth FAQ).
Counseling Compact. California has not joined the interstate Counseling Compact (Counseling Compact map).
Clients physically in California. The BBS requires a valid California license or registration to provide telehealth to anyone physically located in California, with no exceptions. For clients who are located outside California, a licensee may provide services only if they meet that jurisdiction's requirements and that jurisdiction allows telehealth (16 CCR 1815.5(e)); it is the licensee's job to check (BBS telehealth FAQ).
How Prexella handles it. Clients choose telehealth or in office when they book, and the session appears in the same calendar either way. On superbills, Prexella sets place of service 10 (telehealth in the client's home) or 02 (telehealth elsewhere), using the default you choose in billing settings, and 11 for office visits. Prexella does not record where the client is physically sitting for each video session, so confirm and document the client's location at the start of every telehealth session, as the BBS rule requires. California's largest commercial insurers and typical cash-pay rates
Most private-pay clients in California who want reimbursement submit a superbill to one of these carriers. By commercial enrollment at the end of 2024, combining DMHC- and CDI-regulated plans (CHCF California Health Insurers Almanac, 2026 edition):
- Kaiser Permanente, about 48% of commercial enrollment (6.6 million people)
- Anthem Blue Cross (Elevance) and Blue Shield of California, about 14–15% each
- UnitedHealthcare, about 6%
- Aetna (CVS Health), about 4%
- Health Net (Centene), about 4%
Because Kaiser dominates, it is worth asking every new client which plan type they have before promising reimbursement.
Cash-pay fees in California run above the national average. A 2025 guide built from practices' published rates puts typical individual sessions at $150 to $300 (TherapyRoute California guide). Prexella's session credits let clients prepay a package at those rates instead of paying session by session.
Medi-Cal
Medi-Cal is California's Medicaid program, run by the Department of Health Care Services (DHCS). Specialty mental health services are delivered through county plans. Prexella does not bill Medi-Cal; it is built for private-pay and out-of-network practices.
E-signatures and records retention in California
E-signatures. California adopted the Uniform Electronic Transactions Act, which gives an electronic signature the same legal effect as a handwritten one (Cal. Civil Code § 1633.7), alongside the federal ESIGN Act. Prexella's e-signatures store the consent statement, typed name, drawn signature, timestamp, IP address and browser for every signed intake, consent and telehealth form.
Records retention. LMFTs, LCSWs and LPCCs must keep client records for at least seven years from the date therapy ends, and for minors seven years from the client's 18th birthday (B&P Code § 4980.49; § 4993; § 4999.75). Psychologists have the same seven-year minimum from discharge (B&P Code § 2919). Records may be kept electronically.
Running a California practice in Prexella: what changes day to day
Associates and supervision. California has a large population of registered associates (AMFT, ASW, APCC) working toward licensure under supervision. Practices that employ associates can block supervision time on the calendar so clients cannot book over it, and keep each associate's session types and fees on the same branded booking page.
Kaiser questions at intake. Because Kaiser covers nearly half of California's commercial enrollment, many practices ask about the client's insurer during intake and explain reimbursement before the first session. Clients with plans that pay out of network receive a superbill at month end; others pay privately or with prepaid credits.
Telehealth across a very large state. A California license covers clients located anywhere in the state, so a Los Angeles practice can see clients in Sacramento or Fresno by video. Each booking is marked telehealth or office, and the place of service on superbills follows the visit type. The rule that catches clinicians out is travel: when a California client is in another state, that state decides.
Coaching alongside therapy. California's coaching market is large. If you run a coaching business alongside a licensed practice, keep them as separate practices so it is always clear which services are delivered under your license.

Frequently asked questions
Can I see a California client by telehealth while they are traveling in Oregon?
California's BBS says a licensee may treat a client located in another jurisdiction only if the licensee meets that jurisdiction's requirements and that jurisdiction allows telehealth (16 CCR 1815.5). Check the Oregon board's rules before the session; your California license alone does not authorize it.
Is California part of PSYPACT?
No. The California Board of Psychology states that California is not part of PSYPACT. Since January 1, 2026, psychologists licensed elsewhere need Board approval for a temporary out-of-state license to practice in California for up to 30 consecutive days in a calendar year.
Will my Kaiser clients be reimbursed from a superbill?
It depends on the plan. Kaiser is an integrated system and many of its members are in HMO plans, so ask clients with Kaiser coverage to confirm whether their plan pays anything out of network before you rely on a superbill. The same check applies to Anthem, Blue Shield, Aetna and UnitedHealthcare plans.
How long does Prexella keep California client records?
Each practice runs its own Prexella instance and its records stay in its own database until the practice deletes them, so you can meet the seven-year retention rules in the Business and Professions Code. Export your records before closing an account.
Does Prexella bill Medi-Cal?
No. Prexella is built for private-pay and out-of-network practices. Its insurance workspace prepares claims for a biller, but Medi-Cal enrollment and billing through a county plan are separate processes Prexella does not handle.
