Why psychologists need more than a scheduler

A psychology practice carries more moving parts than a typical therapy practice: multi-session evaluations, time-based testing codes, screening instruments repeated across treatment, and clients who pay privately and claim reimbursement themselves. When scheduling, forms, screenings and billing live in separate tools, the evaluation that took four appointments becomes four records you reconcile by hand.

Prexella keeps each client in one record. The booking creates the client, intake and consent forms are signed in the client portal, screenings are scored and saved, and the superbill is built from the sessions that already happened. It is designed for private-pay and out-of-network practices, with an insurance billing workspace for those that also bill payers.

Built for psychologists

Three workflows shape most private-practice psychologists' weeks. Here is how each runs in Prexella.

Workflow 1

Testing and evaluation billed across several dates

A psychological evaluation rarely happens in one sitting. Book the intake, the testing sessions and the feedback session as separate appointments, then build one superbill for the date range. Each date gets its own line: 90791 for the diagnostic interview, 96136 or 96138 for test administration, 96130 and 96131 for evaluation time. Add the units for time-based codes and the client gets one PDF covering the whole assessment.

Workflow 2

Measurement-based care between sessions

Assign PHQ-9 and GAD-7 check-ins in the client portal, along with PCL-5, AUDIT-C, DAST-10 and ASRS where they fit. Scores and severity bands are saved to the client record so you can watch the trend across treatment. Any endorsement of PHQ-9 item 9 routes the client straight to the C-SSRS screener and flags the result for you.

Workflow 3

Telehealth across the states you are licensed in

Clients choose telehealth or an in-office visit when they book, and both sit in the same calendar. Telehealth clients receive your video link by email, and the superbill uses place-of-service 10 or 02 for those sessions automatically. The video itself runs in the HIPAA-ready tool you already use, such as Doxy.me or Zoom for Healthcare.

What practice management software for psychologists should include

The mental health screenings page lists each instrument and how it is scored. The client portal is where clients complete those check-ins, sign forms with ESIGN and UETA e-signatures, message you and reschedule. Online booking checks every slot on the server before confirming it, so evaluation days are never double-booked.

Superbills for psychological testing and therapy

Many psychology clients pay privately and claim out-of-network benefits, and evaluations are where superbills go wrong most often: several dates, mixed codes, and units for time-based testing. A superbill missing an NPI, a tax ID or a diagnosis code gets rejected.

The codes psychologists bill, including psychological testing

Therapy codes are suggested from session length; testing codes are chosen per line. Time-based testing codes are billed as separate lines and units, so a four-hour evaluation is one 96130 line plus 96131 for each additional hour. You choose the codes and the diagnosis; Prexella formats them.

CodeWhat it covers
90791Psychiatric diagnostic evaluation (intake or diagnostic interview)
90834Psychotherapy, 45 minutes
90837Psychotherapy, 60 minutes
96130Psychological testing evaluation, first hour
96131Psychological testing evaluation, each additional hour
96136Test administration and scoring by the psychologist, first 30 minutes
96137Test administration and scoring by the psychologist, each additional 30 minutes
96138Test administration and scoring by a technician, first 30 minutes
96139Test administration and scoring by a technician, each additional 30 minutes

Every Prexella superbill carries your name, credentials, NPI, EIN, license number and taxonomy code, the client's details, the ICD-10 diagnosis you enter, each date of service with its CPT code, units and place of service, and the fees paid. The full field list is on the superbill feature page.

How Prexella compares with TherapyNotes

TherapyNotes is a common choice for psychologists who bill insurance. The full comparison is in Prexella vs TherapyNotes; if you are coming from SimplePractice instead, see SimplePractice alternatives.

TherapyNotes is the stronger choice if most of your revenue is in-network and you want deep claims workflows and clinical documentation. Prexella fits better when most clients pay privately, you want your own brand on the client experience, and you want marketing in the same system.

Pricing

Prexella is priced as one flat fee for the practice rather than per clinician, so adding a postdoc, an associate or a testing technician's caseload does not multiply your software bill. Request a demo and you will get a quote for your practice on the call.

What switching looks like

Export what your current system lets you export, have your services, forms and billing settings set up in Prexella, send new bookings to your new booking page, and let open evaluations finish where they started. The steps are on how to switch to Prexella. For colleagues in your practice, see the pages for therapists and counselors.

Frequently asked questions

Can I put psychological testing codes on a superbill?

Yes. 96130, 96131, 96136, 96137, 96138 and 96139 are available alongside the psychotherapy codes. Each date and code is its own line, so a multi-day evaluation fits on one superbill.

Which screening instruments are built in?

PHQ-9, GAD-7, PCL-5, AUDIT-C, DAST-10, ASRS and the C-SSRS screener. Scores and severity bands are saved to the client record. They are screening tools, not diagnoses, and you interpret the results.

What happens when a client endorses suicidal ideation on the PHQ-9?

Any answer above zero on item 9 sends the client straight to the C-SSRS screener and flags the result on their record. Your own safety protocol still applies; Prexella surfaces the result, it does not replace clinical follow-up.

Do you support psychologists who also take insurance?

Yes. The insurance billing workspace turns sessions into claims and sends them to an in-house or outsourced biller, who works in a separate portal that only shows the claim itself.

Can I offer telehealth to clients in other states?

Prexella schedules telehealth sessions and records the visit type, but where you may practice depends on your licenses and your state's rules, including PSYPACT participation. Check those before booking clients outside your licensed states.
