Prexella vs Carepatron

Prexella vs Carepatron compares two systems that both let a practice put its own brand forward: Carepatron is a low-cost, per-user all-in-one with a free plan and white labeling on its top tier, and Prexella is a flat-fee, white-label system on your own domain built for private-pay practices. Here is the honest comparison, with Carepatron pricing checked on 10/10/2026.

The short answer

Choose Carepatron if you want the lowest possible cost to start, including a genuinely usable free plan, you are comfortable paying per user as you grow, and built-in telehealth video matters to you.

Choose Prexella if you run a private-pay therapy, counseling or coaching practice and want your brand on your own domain, one-click superbills for out-of-network clients, prepaid session credits, a marketing suite in the same system, and one flat fee no matter how many clinicians you add.

Prexella vs Carepatron: 12 criteria side by side

Pricing for a three-clinician practice

Here is the math for an owner and two associates. Carepatron bills practitioners per user (Carepatron pricing, verified 10/10/2026).

Line Plus, monthly Advanced, monthly
3 users at list price ($31 / $39) $93 $117
Same, during the 50%-off first six months $46.50 $58.50
Optional: e-prescribe ($39 × 3) $117 $117

At list price, that is $1,116 a year on Plus or $1,404 a year on Advanced, the plan that includes white labeling. Carepatron also lists 20% off for yearly billing without showing the per-month figure. Electronic claims start at 25 cents each and extra SMS messages are 5 cents, so a practice that bills insurance will see small usage charges.

Prexella. One flat fee covers the practice, whether you have one clinician or five, and it is quoted for your practice on a demo call. Carepatron is likely to be cheaper for a small team at list price, so the decision usually comes down to what is included: your own domain, superbills, prepaid credits and marketing, against Carepatron's built-in video and free entry point.

What clients see in each system

Booking and paying. A Prexella client books on your own domain: they pick a service, choose telehealth or in office, take an open time and pay by card in one flow. Every slot is checked on the server before it is confirmed, so double bookings and past-date bookings cannot happen. Clients can also buy prepaid session credits or a multi-session package. Carepatron offers an online booking embed you can place on your own site.

The portal. Both systems give clients a portal. Prexella's client portal is where clients sign intake and consent forms with an ESIGN and UETA audit trail, message you, buy credits, complete PHQ-9 and GAD-7 check-ins, keep a journal and work through workbooks. Carepatron includes a client portal even on its free plan.

Branding. Prexella clients only see your practice's name and domain. Carepatron offers custom branding on Plus and white labeling on Advanced.

What a practice owner's week looks like

Monday: open the dashboard to see the week's sessions, clients whose session credits are running low, and forms still waiting for a signature. Prexella emails clients automatically when their credit balance runs low and the day before the last session of a package, so you are not writing those reminders yourself.

During the week: every client gets a reminder the day before each session, and telehealth clients get the video link in the same email. Recurring clients keep their weekly slot without being rebooked, and new clients book, pay and sign their paperwork without a phone call.

Month end: generate superbills for out-of-network clients in one click each, review income in Finances, and check which marketing channels brought in new clients this month. In Carepatron, the same week runs inside one low-cost workspace with built-in video, which is its main appeal for small teams.

Where Prexella wins

1. Your own domain, on a dedicated instance

Both products let a practice put its brand forward, but Prexella goes further: the booking page, client portal and every confirmation, reminder and superbill email run on your own domain, on an instance dedicated to your practice. Carepatron's pricing page lists white labeling on Advanced but does not list a custom domain. See white-label on your own domain.

2. One price, not one per user

Carepatron's paid plans are billed per user, so every clinician you add raises the bill, and its white-label plan is the more expensive of the two. Prexella is one flat fee for the practice, so hiring an associate or a part-time clinician does not change it.

3. Superbills, credits and marketing for private-pay practices

Prexella builds a superbill in one click from a client's completed sessions, with CPT, ICD-10, NPI, EIN and place of service filled in, so out-of-network clients can claim reimbursement. Superbills are not listed on Carepatron's pricing page. Prexella also sells prepaid session credits with automatic low-balance reminders, and its marketing suite keeps the ad results you record, video scripts and client newsletters next to the calendar. Carepatron lists email sends and a booking embed.

Where Carepatron wins

1. A free plan that covers the basics

Carepatron's free plan includes unlimited clients, telehealth, 1 GB of storage and a client portal. For a practitioner starting out, that is a real way to run a practice at no cost. Prexella has no free plan; it is set up for you after a demo.

2. Built-in telehealth

Carepatron includes telehealth video on every plan, including Free, and adds group telehealth on Plus. Prexella schedules telehealth and sends the join link, but the video runs in a separate HIPAA-ready tool you pay for yourself.

3. Lower cost for small teams, with e-prescribing and claims

At list price, three users on Carepatron cost $93 to $117 a month, and current promotions halve that for six months. It also offers e-prescribing at $39 per user and electronic claims from 25 cents. Prexella does not offer e-prescribing and hands insurance claims to your biller to submit. Reviewers on G2 do cite limited or missing features as Carepatron's top drawbacks (G2 comparison), so check the specific workflows you need during the trial.

Questions to ask before you choose

A demo or trial is the best place to test claims, including ours. Questions worth asking Carepatron:

  • Which features we need are on Plus versus Advanced, and what is the price after the six-month promotion ends?
  • Does white labeling on Advanced include running the portal on our own domain?
  • How are superbills produced for out-of-network clients, and what fields do they include?
  • Can clients buy prepaid packages or session credits?
  • How many email sends a month will our reminders and newsletters use on each plan?

And questions worth asking us on a Prexella demo:

  • What is the flat fee for our practice, and what does setup include?
  • How will our client records be moved, given what our current system lets us export?
  • Which video tool should we pair with Prexella for telehealth, and does it sign a BAA?

Verdict

Prexella vs Carepatron is closer than most comparisons, because both take branding seriously. Carepatron wins on entry cost, a free plan and built-in video, and it is a sensible choice for a budget-conscious solo or small multi-disciplinary practice. Prexella wins for private-pay practices that want their own domain, superbills, prepaid packages and marketing in one system, and a flat fee that does not climb with every hire.

Frequently asked questions

Is Prexella cheaper than Carepatron?

Usually not for a small team. Carepatron has a free plan, and its paid plans list at $31 and $39 per user a month, currently 50% off for six months. Three users on the Advanced plan, which includes white labeling, list at $117 a month. Prexella is one flat fee for the practice, quoted on a demo call, so compare the totals for your headcount.

Both offer white-labeling. What is the difference?

Carepatron lists custom branding on Plus and white labeling on Advanced; a custom domain is not listed on its pricing page. Prexella runs booking, the client portal and every email on your own domain with your practice name, on an instance dedicated to your practice.

Does Prexella have a free plan like Carepatron?

No. Prexella is set up for you after a demo and priced as one flat fee. Carepatron's free plan includes unlimited clients, telehealth, 1 GB of storage and a client portal, which is hard to beat for a practitioner just starting out.

Does Prexella include telehealth video?
