What to include in a therapist intake form
A good intake packet does three jobs: it gets the practical details you need to schedule and bill, it gives you a starting picture of the client, and it records the consents and policies the client agreed to before treatment began. This template is organized around those jobs:
- Client information: legal and preferred name, pronouns, date of birth, contact details and whether you may leave voicemails or texts.
- Emergency contact: a name, relationship and phone number.
- Insurance and payment: plan, member ID and policyholder details, so you can issue superbills for out-of-network reimbursement.
- Presenting concerns: what brings the client in, for how long, and what they want to change.
- History: prior treatment, medications and prescriber, medical conditions, substance use, risk history and primary care provider.
- Consent to treat: benefits and risks of therapy, confidentiality and its legal limits, and crisis resources such as 988.
- Telehealth consent: technology risks, privacy at the client's end, and confirming the client's location at each session.
- Acknowledgment of the Notice of Privacy Practices: a signed acknowledgment that the client was offered your notice.
- Cancellation and payment policy: your notice window, late-cancellation fee and when payment is due.
- Signature: the client's signature and date, plus a parent or guardian line for minors.
The privacy-practices acknowledgment matters because HIPAA asks a provider with a direct treatment relationship to make a good faith effort to obtain a written acknowledgment that the client received the notice (45 CFR 164.520(c)(2)(ii)). The template includes that acknowledgment; you still need your own Notice of Privacy Practices to give the client.
How to adapt the template for your practice
The template is a starting point, not a finished legal document. Before you send it to clients:
- Fill in the blanks. Add your practice name, the cancellation notice window (24 or 48 hours is common) and the late-cancellation fee.
- Match your services. If you see couples or families, add a section naming every participant and how records are shared between them. If you work with minors, expand the guardian section and explain what parents will and will not see.
- Write your own confidentiality limits. The template lists the common exceptions in plain terms; your board may expect specific wording, such as duty-to-warn language.
- Pair it with your Notice of Privacy Practices. The acknowledgment only makes sense if the client was actually offered your notice.
- Decide how you will store it. Paper forms need a locked file; scanned forms and PDFs belong in a system with access controls, not a shared drive.
Send the packet before the first session rather than handing it over in the waiting room. Clients answer history questions more carefully at home, and you can read the answers before you meet them.
State consent nuances
Intake forms are not one-size-fits-all across states. Before you use this therapist intake form template, check three things with your licensing board:
- Minors. The age at which a minor can consent to outpatient mental health treatment without a parent varies by state, and so do the rules about what parents can see.
- Telehealth. Some states require a specific, documented telehealth consent, and every state requires that you are licensed or otherwise authorized where the client is located during the session.
- Records. How long you must keep intake forms and other client records depends on your state and license type.

Frequently asked questions
Does this template make my practice HIPAA compliant?
No form can do that on its own. The template includes an acknowledgment of your Notice of Privacy Practices, which HIPAA asks providers to make a good faith effort to obtain, but compliance also depends on how you store, share and protect client records. Have your own policies reviewed by a qualified professional.
Can I edit the template?
Yes. Download the editable version and open it in Microsoft Word, Pages or Google Docs (upload it to Google Drive and open it with Google Docs). Add your practice name, cancellation window and fees, and adjust the consent wording for your state.
Do I need a separate telehealth consent?
Many states expect documented informed consent for telehealth, and the rules differ by state and license. The template includes a telehealth consent section you can keep, expand or replace. Check your licensing board's rules for your state.
Why do you ask for my email?
