Practice Management Software for Private Practices in the San Francisco Bay Area

Prexella is practice management software for San Francisco, Oakland and Peninsula private practices that run mostly out of network: superbills, evening telehealth, prepaid packages and a client portal on your own domain.

A high-fee, out-of-network market

The Bay Area has some of the highest household incomes in the country, which is why so many clinicians here stay entirely out of network. Median household income was $139,801 in San Francisco County, $129,130 in Alameda County and $168,154 in Santa Clara County in 2024 (U.S. Census Bureau ACS 2024 via Census Reporter). Practices price accordingly, and clients expect to claim part of the fee back from their insurer.

Commutes across the bridges and long workdays at large employers push many sessions to early mornings, lunch hours and evenings, often by video.

Three Bay Area workflows

Monthly superbills for employer PPO clients

The core Bay Area billing routine is a superbill at month end. In Prexella, pick the client and the month; completed sessions load with CPT codes suggested from their length (90834 for 45 minutes, 90837 for 60), your NPI, EIN and license number from billing settings, and the ICD-10 code you enter. The client downloads a PDF and submits it. See superbills.

Split schedule: Mission or Rockridge office, evening video

Clients pick telehealth or office at booking, and both sit in the same calendar, so a clinician can keep office hours two days a week and see clients by video in the evenings. Reminders go out the day before, with the join link for video sessions. Confirm the client's location at the start of each telehealth session, since California only permits sessions with clients in other states when those states allow it.

Assessments and longer evaluations

Psychologists running ADHD or learning evaluations bill across several dates. Prexella's superbills include the psychological testing codes (96130–96139) as separate lines, so a client can submit a whole evaluation at once. Booking packages that schedule weekly dates in one checkout work for multi-session assessments too.

Your own brand in a crowded market

Every client-facing page and email runs on your own domain with your logo. Bay Area clients often compare several practices before booking; a clean online booking page that takes payment and fills in intake forms helps you win the first session.

Pricing and reimbursement conversations

Bay Area clients usually ask two questions before booking: what the session costs, and what they will get back. Prexella helps with both.

Your fees appear on the booking page for each session type, so there are no surprises at checkout. For the reimbursement question, ask clients to call their insurer and confirm three things: whether their plan covers out-of-network outpatient mental health, the out-of-network deductible, and the percentage reimbursed after it. Clients who know those numbers are far more likely to stay in private-pay therapy.

At the end of each month, the superbill does the rest. Clients with large-employer PPO plans submit it through their insurer's member portal. If your practice has a group NPI, Prexella prints it alongside the individual NPI, along with the taxonomy code and license number from your billing settings.

For practices that prefer prepayment, clients can buy a package of session credits once and use them across the month, which works well for weekly clients who also want a single superbill at month end.

Frequently asked questions

My clients work for large tech employers. Will they get reimbursed?

Many have employer PPO plans, but benefits vary by employer and plan. Ask each client to check their out-of-network mental health benefit and deductible. Prexella then gives them a superbill with the identifiers and codes their insurer needs.

Can I offer evening telehealth slots only?

Yes. Open slots are set by you, and each service can be offered as telehealth, in office or both, so a practice can keep office hours by day and video sessions in the evening.

Does Prexella work for psychologists doing assessments?

Yes. The superbill tool includes psychological testing codes 96130 to 96139, billed as separate lines with units, alongside the psychotherapy codes.
