Pricing for a three-clinician practice

Upheal's pricing page lists four plans (Upheal pricing, verified 10/10/2026):

For groups, the page says practices can buy "per-seat" but does not list the seat price. That means a three-clinician total cannot be worked out from public information, and we are not going to guess it. To make a fair comparison, ask Upheal for a group quote and compare it with Prexella's.

Two things are clear from the page. First, on the pay-as-you-go EHR plan the bill scales with sessions: at $1 a session, a clinician seeing 25 clients a week would pay roughly $100 a month, if per-session pricing applies to each clinician in a group. Second, usage charges sit on top: insurance claims are 30 cents a submission, eligibility checks 15 cents each, and online payments 2.9% plus 30 cents.

Prexella. One flat fee covers the practice, whether you have one clinician or five, and it is quoted for your practice on a demo call. It does not rise with session volume or headcount.

What clients see in each system

Booking and paying. A Prexella client books on your own domain: they pick a service, choose telehealth or in office, take an open time and pay by card in one flow. Every slot is checked on the server before it is confirmed, so double bookings and past-date bookings cannot happen. Clients can also buy prepaid session credits or a multi-session package.

The portal. Both products give clients a portal. Prexella's client portal is where clients sign intake and consent forms with an ESIGN and UETA audit trail, message you, buy credits, complete PHQ-9 and GAD-7 check-ins, keep a journal and work through workbooks. Upheal lets you apply your logo and brand colors to forms, telehealth sessions and its client portal.

Branding. Prexella runs everything on your own domain, so clients only see your practice. Upheal's branding covers your logo and colors; a custom domain is not listed on its pricing page.

What a practice owner's week looks like

Monday: open the dashboard to see the week's sessions, clients whose session credits are running low, and forms still waiting for a signature. Prexella emails clients automatically when their credit balance runs low and the day before the last session of a package, so you are not writing those reminders yourself.

During the week: every client gets a reminder the day before each session, and telehealth clients get the video link in the same email. Recurring clients keep their weekly slot without being rebooked, and new clients book, pay and sign their paperwork without a phone call.

Month end: generate superbills for out-of-network clients in one click each, review income in Finances, and check which marketing channels brought in new clients this month. In Upheal, the same week centers on AI-drafted progress notes after each session, which is the time saving its users buy it for.

Where Prexella wins

1. The business side of a private-pay practice

Prexella is built around how cash-pay practices earn: online booking with card checkout, prepaid session credits with automatic low-balance reminders, multi-session packages, and one-click superbills from completed sessions for clients claiming out-of-network benefits. Upheal lists superbills on its EHR plans but not prepaid packages, and its focus is documentation.

2. Marketing in the same system

Upheal's pricing page does not list marketing tools. Prexella's marketing suite lets you record and review Meta and Google ad results, plan video scripts and send branded client newsletters from the same dashboard as your calendar. For a practice that has to find its own clients, that is where growth comes from.

3. Your own domain and a predictable bill

Prexella is white-label: booking, the portal and every email run on your domain. And because it is one flat fee, your software cost does not move with session volume or with each clinician you hire, which is easier to budget for than per-session or unlisted per-seat pricing.

Where Upheal wins

1. AI progress notes

This is Upheal's core product. Its free plan includes text-to-note and dictation, AI Notes is $39 a month for unlimited sessions, and EHR Pro adds AI Assistant Pro. Prexella keeps session notes but does not generate them. If documentation is eating your evenings, Upheal addresses that directly and Prexella does not.

2. Built-in telehealth

Upheal includes telehealth in every plan, including Free. Prexella schedules telehealth and sends the join link, but the video itself runs in a separate HIPAA-ready tool you pay for.

3. A free start and pay-per-session pricing

Upheal's free plan and $1-per-session EHR make it easy to start with a small caseload and pay in proportion to your volume. Prexella is set up for you after a demo and has no free plan. Verdict

Prexella vs Upheal is less a head-to-head than a question of which problem you are solving first. If it is documentation, Upheal's AI notes are the point of the product, and Prexella does not compete there. If it is the business of a private-pay practice (bookings, payments, packages, superbills, marketing and your own brand), Prexella covers far more of it. Some practices will reasonably use an AI note-taker alongside Prexella.
