The Long Island private-practice landscape
Nassau and Suffolk together hold one of the largest licensed-clinician workforces in New York. As of July 1, 2026, the Office of the Professions counted 1,108 licensed mental health counselors in Nassau County and 1,043 in Suffolk County, the highest LMHC counts outside Brooklyn (NYSED LMHC statistics).
Distance is the defining feature. A practice in Garden City or Huntington can draw clients from the whole island, and a drive from the East End can take longer than the session. Long Island practices have leaned into telehealth for far-flung clients while keeping an office for those nearby.
Three Long Island workflows in Prexella
1. Video for the East End, office for the North Shore
Let clients choose. A client in Riverhead books video most weeks; a client in Mineola comes in. On your online booking page each client chooses telehealth or in office, and the confirmation and reminder emails include the video link only for video sessions. The day's schedule shows which hours you need to be at the office.
2. Year-round recurring slots
Long Island clients often keep the same evening slot for months. Recurring weekly slots hold the time automatically, with a reminder the day before every session. If a client cancels inside 48 hours, the late-cancellation credit penalty applies without a conversation.
3. Superbills for out-of-network clients
Clients with employer plans who choose an out-of-network counselor submit a superbill themselves. Prexella builds it from completed sessions with your New York license number, NPI, EIN, diagnosis and CPT codes such as 90834 and 90837, and marks each session's place of service as office or telehealth.
Group practices on the island
Many Long Island groups have several counselors and social workers under one name. Prexella is one flat fee for the practice, so adding a clinician does not add a seat charge, and every clinician's work appears under the practice's own brand.
What Long Island practices set up first
Long Island practices often have a longer list of session types than city practices because they serve families, couples and individuals from a wide area. A typical setup on the first call:
- Intake at a longer length, billed as 90791 on superbills.
- Individual follow-ups at 45 and 53 minutes, suggested as 90834 and 90837 from the length.
- Family sessions with the client present, suggested as 90847.
- Group sessions, if you run them, suggested as 90853.
Each session type has its own fee, and clients see only what is open when they book.
Summer schedules. East End practices see seasonal shifts. Block the weeks you are away, and clients with recurring slots can reschedule individual weeks themselves in the portal. Telehealth lets seasonal residents keep their sessions from wherever they are, provided you are licensed or authorized where they are during the session.
Two offices. If you keep offices in both Nassau and Suffolk, block your hours at each and name the location in the session type so clients book the right one.
Pricing and switching. Prexella is one flat fee for the practice, quoted on a demo call, with no per-clinician seats. Most Long Island groups move over in one billing cycle; see how to switch.

Frequently asked questions
Most of my Suffolk clients are an hour away. Can they do video most weeks and come in once a month?
Yes. Clients choose telehealth or in office each time they book or reschedule, and the superbill applies the correct place-of-service code to each session.
Can I keep a client on the same weekly slot all year?
Yes. Move them onto a recurring weekly slot and Prexella holds it, sends a reminder the day before each session and lets them reschedule one week from the portal.
Can Prexella also handle my in-network clients?
Prexella's insurance billing workspace turns sessions into claims and hands them to your in-house or outsourced biller, who submits them through their own clearinghouse. It is built mainly for private-pay and out-of-network work.
