Boston's practice landscape
Boston has one of the densest concentrations of therapists, hospitals and universities in the country, and a large share of private practitioners stay out of network. Median household income in the city was $97,791 in 2024 (U.S. Census Bureau ACS 2024 via Census Reporter), close to the Massachusetts median of $104,828. Many clinicians trained in Boston's hospital systems open private practices in Back Bay, the South End and Jamaica Plain, often as small groups of LICSWs and LMHCs.
Three Boston workflows
Out-of-network superbills as the default
Many Boston clients pay their therapist directly and claim reimbursement. Prexella builds each superbill from completed sessions: practice and provider details, NPI, EIN, license number, ICD-10 diagnosis, CPT code suggested from session length, place of service and amounts paid. Blue Cross Blue Shield of Massachusetts and Harvard Pilgrim are among the largest local carriers (see the Massachusetts page).
Students who leave for the summer
Boston's universities mean many clients are students whose location changes with the academic calendar. Massachusetts has not joined PSYPACT or the Counseling Compact, so when a student goes home for the summer, the home state's rules decide whether video sessions can continue. Confirm the client's location at the start of each telehealth session; Prexella does not record it automatically.
Small groups of LICSWs and LMHCs
Groups need one booking page and one set of policies across clinicians. Prexella keeps session types, fees, forms and booking rules at the practice level, on the practice's own domain, at one flat fee rather than per clinician.
The client experience
Clients book, sign intake and consent forms, reschedule and message you in the client portal, on your domain and under your name. Telehealth clients receive their join link by email; the video runs in your existing HIPAA-ready tool.
What Boston practices set up first
Superbill details. Add each clinician's NPI, license number and taxonomy code, plus the practice EIN and any group NPI, to billing settings. Every superbill uses them, so clients submitting to Blue Cross Blue Shield of Massachusetts or Harvard Pilgrim are not sent back for missing identifiers.
Telehealth and office hours. Decide which session types are offered by video, in the office, or both, and open slots accordingly. Telehealth clients receive their join link in the confirmation and the day-before reminder.
Paperwork. Intake, informed consent, the privacy notice and telehealth consent are sent after booking and signed in the portal with a full audit trail.
Academic-year planning. Before May and December, review which student clients will be leaving Massachusetts and plan referrals or pauses. Prexella keeps their full history, signed forms and superbills on one record if they return in the fall.
For Boston practices, the result is less administrative time per client in a market where many clients pay out of pocket and expect a professional experience from first contact.
Telehealth inside Massachusetts
Because a Massachusetts license covers clients anywhere in the state, Boston practices can see clients in Worcester or on the Cape by video. Bookings record whether each session is telehealth or office, and superbills follow the visit type.
Nearby
Massachusetts licensing, telehealth, insurer and records rules are on the Massachusetts page. Nearby metros: Cambridge and Newton. Clinical social workers can also read practice management for LCSWs.

Frequently asked questions
My college student client goes home to Ohio for the summer. Can we keep meeting?
Only if Ohio's rules allow it. While the student is located in Ohio, Ohio's licensing rules apply, and Massachusetts has not joined the Counseling Compact or PSYPACT. Check the Ohio board before continuing by video.
Can my group practice include LICSWs, LMHCs and psychologists?
Yes. Prexella stores each provider's license number, NPI and taxonomy code in billing settings, and session types and fees are set for the practice.
Do Boston clients on Blue Cross get reimbursed?
That depends on the client's plan. Ask them to confirm their out-of-network mental health benefit, then give them a Prexella superbill.
