Why practices look for TherapyNotes alternatives

TherapyNotes is a well-regarded behavioral-health EHR with strong claims workflows and phone support. People rarely search for TherapyNotes alternatives because it stopped working. They search because the cost or the fit changed. These are the reasons we could source.

The December 2025 price increase. TherapyNotes announced on October 30, 2025 that from December 1, 2025 Solo would cost $69 a month, and Group $79 a month for the first clinician plus $50 for each additional clinician (TherapyNotes pricing update). Those are the current list prices (TherapyNotes pricing, verified 10/10/2026).

Per-clinician fees. On Group, every additional clinician adds $50 a month. Non-clinical users such as billers and schedulers are unlimited, which is generous, but a practice hiring associates still sees its bill grow with each clinician.

Usage charges. Electronic claims, ERA claims and eligibility requests are 14 cents each, paper claims are $1, and text or voice reminders are 14 cents each; email reminders are free. Premium telehealth is $15 per clinician a month, the TherapyFuel AI tool is $40 per clinician a month, and e-prescribing is $65 per prescriber a month (all verified 10/10/2026 on the TherapyNotes pricing article). None is unreasonable on its own, but they add up for a busy practice.

No white-label and little marketing. TherapyNotes lists a "custom client portal", but custom branding on your own domain is not listed on their pricing page. Marketing is limited to a free TherapySearch directory profile. For private-pay practices that grow through their own brand, that is a real gap.

For reference, TherapyNotes itself: $69/mo Solo, $50 per additional clinician on Group, basic telehealth included and premium at $15, superbills and white-label not listed on their pricing page, marketing limited to a directory profile.

Each alternative, and who should choose it

Prexella

Prexella is a white-label practice management system for private-pay therapists, counselors, psychologists and coaches. It combines online booking with card checkout, prepaid session credits, one-click superbills, a client portal with e-signed forms and PHQ-9 and GAD-7 check-ins, and a built-in marketing suite, all on your own domain. It is one flat fee for the practice, set up for you, and quoted on a demo call. Reminder emails are part of the system rather than billed per message.

What it does not do: Prexella does not host video itself; it schedules telehealth and sends the join link, and the session runs in your own HIPAA-ready video tool. It does not submit claims to a clearinghouse directly; its insurance workspace prepares claims and hands them to your biller. And it offers fewer clinical documentation tools than TherapyNotes.

Choose Prexella if most of your clients pay privately or out of network, you want the practice to carry its own brand, and you are adding clinicians without wanting a per-seat bill.

SimplePractice

SimplePractice is the best-known all-in-one EHR for solo and small-group therapists. Plans are $49, $79 and $99 a month, with additional clinicians starting at $74 each on Plus. Telehealth is included in every plan. Paid add-ons include the Care Aide AI suite at $59 a month plus $49 per additional clinician, AI Note Taker at $35, and ePrescribe at $49 plus an $89 setup fee (SimplePractice pricing, verified 10/10/2026). Choose SimplePractice if you want a complete clinical EHR with built-in video and AI documentation tools and do not mind per-clinician pricing.

Jane App

Jane is built for multi-discipline clinics. Plans are $59, $79 and $99 a month, with additional full-time practitioners at $35 or $40 and part-time at half that. Superbills and CMS-1500s are included, insurance billing is a $20-a-month add-on on Practice and Thrive, and Jane Websites is $59 a month per clinic (Jane pricing, verified 10/10/2026). Jane states that it does not offer a free trial.

Choose Jane if your practice mixes therapy with physiotherapy, chiropractic or massage and you want one booking system for all of them.

Practice Better

Practice Better is aimed at nutrition, functional medicine, coaching and wellness practitioners. There is a free plan for up to three clients and paid plans from $35 to $155 a month, with additional practitioners at $50 on Team. Superbills and telehealth are included from Starter, and Plus adds portal branding and a personalized URL (Practice Better pricing, verified 10/10/2026).

Choose Practice Better if you sell structured programs and your clients are mostly wellness rather than therapy clients.

Sessions Health

Sessions Health is the low-cost therapy EHR in this list: free for up to three clients, then $39 a month for the first practitioner and $29 for each additional one. Telehealth is a $10 add-on per practitioner, electronic claims start at 25 cents, and automated superbills are listed (Sessions Health pricing, verified 10/10/2026). Capterra reviews skew positive.

Choose Sessions Health if you want to keep an insurance-capable EHR and pay less for it.

Healthie

Healthie targets nutrition, wellness and virtual-first care, including companies building on its API. Plans run from $19 a month for 10 active clients to $149 or more for groups, with team members at $50 a month. Telehealth is included and superbills are listed as a premium feature (Healthie pricing, verified 10/10/2026).

Choose Healthie if you run a virtual-first nutrition or wellness business.

Carepatron

Carepatron has a free plan with unlimited clients and telehealth, and paid plans billed per user at $31 and $39 a month, currently discounted for six months. It lists white labeling on its Advanced plan and electronic claims from 25 cents (Carepatron pricing, verified 10/10/2026).

Choose Carepatron if you want the lowest cost to start and are comfortable with per-user pricing as you grow.

Upheal

Upheal began as an AI note-taker and now offers EHR plans: free, $39 a month for unlimited AI notes, $1 per session for the EHR, or $99 a month for EHR Pro. Telehealth is included, superbills are on the EHR plans, and insurance claims are 30 cents per submission (Upheal pricing, verified 10/10/2026).

Choose Upheal if AI progress notes are the main thing you want to speed up.

TherapyAppointment

TherapyAppointment prices by usage: $10 a month until you book more than 10 sessions, $39 until 39, then $59. Group practices pay $59 plus $39 per additional clinician. Electronic claims are 15 cents through Office Ally and telehealth is a $15 add-on (TherapyAppointment pricing, verified 10/10/2026). Reviewers on Capterra describe the interface as functional but dated.

Choose TherapyAppointment if you are starting with a small caseload and bill insurance.

How to choose

  1. If cost is the reason: add up next year's clinician count, telehealth, AI tools and per-claim or per-reminder fees for each option. A flat fee only wins once you have more than one or two clinicians; request a Prexella quote and compare the real totals.
  2. If you are mostly in-network: stay with a claims-first system. Sessions Health, SimplePractice and TherapyAppointment are the closest swaps.
